Salary Range: $32.00 – $48.00Department DetailsSchedule: Full-time position consisting of four 9-hour shifts per week during clinic hours.Autonomy, great team work, practice at the top of your scopeJob SummaryThe Registered Nurse (RN) is responsible for utilizing the nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation) to provide individualized nursing care to dialysis patients. Position Overview – Home Dialysis NurseWe are seeking a dedicated and compassionate nurse to join our Home Dialysis team. This role is responsible for the coordination of care, patient assessment, education, and various other nursing interventions for patients on home dialysis. Our program includes both Peritoneal Dialysis (PD) and Home Hemodialysis (HHD).Key Responsibilities:Educate and train patients and caregivers on performing dialysis treatments safely and effectively at homeConduct home visits as needed (mileage reimbursed or company vehicle provided)Provide ongoing support and care coordination for home dialysis patientsParticipate in a rotating on-call schedule to provide remote support for patients after hoursCollaborate with physicians and inter-professional team members to plan, implement, and evaluate individualized dialysis care plansDocument patient history, symptoms, medications, and care provided in compliance with standards and regulationsUtilize technology to enhance care quality and delegate appropriately while maintaining nursing accountabilityQualifications:Dialysis experience is preferred, though we are open to training the right candidate with a strong clinical background and a passion for patient educationMust practice within the full scope of nursing knowledge and expertise and follow all age- and population-specific care standardsAdheres to the Sanford Professional Nursing Practice framework, including the ANA Scope and Standards of Practice and Code of EthicsThis position offers a meaningful opportunity to build long-term patient relationships, foster independence through home therapy, and contribute to high-quality, patient-centered care in a supportive and collaborative environment.QualificationsBachelor’s Degree in nursing preferred.Graduate from an accredited nursing program required, including, but not limited to, American Association of Colleges of Nursing (AACN), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA).Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state.
