- Provides Medicare visits for HHC patients
- Is familiar with Medicare requirements and is able to perform within a given
scope of practice * Provides ongoing support and evaluation of progress toward stated health goalsRevises the healthcare plan as needed in collaboration with the patient and
healthcare team. * Possesses excellent communication and collaboration skills.
- Determines the need for and completes appropriate referral processes
- Maintains EHR documentation and data. Completes all documentation in a timely manner
- Meets with supervisor regularly to provide patient updates
Qualifications:
Graduate of an accredited School of Nursing (BSN or equivalent Preferred).
Active Registered Nursing license in Maryland / Virginia
Current CPR certification
Clinical experience: 1 year (minimum) direct patient care within the last 3 years [preferred]
Candidate possessing strong clinical skills to manage the care of technologically dependent patients with trachs / feeding tubes / ventilators is preferredBenefits may include:Health insurance
