Key Responsibilities:
- Conduct in-home assessments and create personalized physical therapy plans for patients.
- Provide one-on-one physical therapy sessions, tailored to individual patient needs.
- Collaborate with healthcare professionals, including nurses and physicians, to deliver comprehensive patient care.
- Educate patients and their families on exercises and techniques for ongoing care.
- Document patient progress and maintain accurate records.
Qualifications:
- Valid Physical Therapist license in the state of Illinois.
- Strong clinical skills and the ability to provide high-quality patient care.
- Compassion, empathy, and excellent communication skills.
- Reliable transportation and a willingness to travel to patients’ homes.
- Prior experience in home health care is a plus, but not required.
