Responsibilities

Collaborates with multidisciplinary teams, provides counseling and/or psychotherapy, coordinates patient discharge planning, complies with required standards for medical/legal responsibilities, and serves as a resource to other allied and administrative staff within Outpatient Social Work and the Department of Care Management.

At Mayo Clinic Florida, the heart VAD transplant social worker plays a vital role in supporting patients and their families throughout the transplant process. They provide psychosocial assessments, emotional support, and counseling to help patients cope with the challenges of waiting for, undergoing, and recovering from heart transplantation. This licensed clinical social worker collaborates with a multidisciplinary medical team to evaluate patients’ social support systems, financial resources, and mental health status, ensuring readiness for transplant and long-term adherence to post-operative care. Additionally, they assist with resource coordination, including housing, transportation, insurance, and access to community services, while advocating for the patient’s overall well-being and quality of life.  Teamwork is an important value at Mayo Clinic and the heart transplant program demonstrates this with every patient.  The Heart Transplant Vad Social Worker facilitates a support group, education programs, community events and more.  As a member of the outpatient social work team, you will have access to a strong team of clinical social workers,  committee and special project opportunities, serve as preceptor in our internship and fellowship program, and work towards obtaining academic rank.  

Qualifications

Requires a Master of Social Work (MSW) or Master of Science in Social Administration (MSSA) degree from an accredited social work program.Candidates must have an advanced knowledge of mental illness, personality development, knowledge of the types of treatment, and skills in conducting individual, group, and family therapy. Must have knowledge of adjustment to medical diagnosis’ and chronic disease. Strong communication skills are needed to interact with patients, patient’s family members, consultants, residents, hospital personnel, community agencies, and other resources. Must also have psychosocial diagnostic skills and the ability to communicate effectively in both verbal and in written form.Licensure (LCSW) to practice independently at the clinical level as required by applicable state requirements.

$70,116 – $105,289 / year; Education, experience and tenure may be considered along with internal equity when job offers are extended.
